Job Description:
Position is located with the Division of Community Health Services, Health Education Center for Wellness/Primary Care Branch, Northern Navajo Medical Center, Shiprock, New Mexico. The Supervisory Nurse (Case Management) is responsible for supervising, managing and directing the nursing care and nursing services for the Primary Care Case Management Program and other areas where patient education and care is provided.

BASIC REQUIREMENT(S): Individual Occupational Requirements Note: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education(external link), Council on Accreditation of Nurse Anesthesia Educational Programs(external link), Accreditation Commission for Midwifery Education(external link), or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools. Areas of specialties for positions include, but are not limited to, Acute Care, Anesthesia, Community Health, Family, Geriatric, Home Health, Midwifery, Neonatal, Oncology, Pediatric, Psychiatric, Allergy and Immunology, Women's Health, Cardiovascular, Dermatology, Occupational Health, Orthopedic, Respiratory, or other specialties identified for the position. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination(external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. Basic Requirements for the GS-12 (or equivalent) Grade Level In addition to the mandatory license and education described above under Basic Requirements applicants must have at least one or more full years of professional nursing experience that is equivalent to the next lower grade level. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Experience and knowledge of Case Management and Care Coordination processes to develop policy and procedures for a case management department. Experience in the development and administration and supervision of the Case Management Department of the clinical component and appropriately involves the case managers and care coordinators. Experience in establishing, implementing, and managing a model of best practice that is appropriate for the needs of the Service Unit. Experience in implementing goals and plans to assist/support case managers and care coordinators to implement quality of care and managed care monitors and controls in health care activities. Works on a daily basis with the Health Education Center Supervisory Physician, Transforming Primary Care leaders, Department chiefs and other key staff members with responsibility for fully participating in top management discussion, decisions and policy making, and sharing accordingly in management decisions/actions affecting diabetes care. Participates in decisions which affect the ability of the service unit to provide adequate patient care. Establish and implement long and short-term policies and program plans for the nursing case management education and services. Responsible for case management nursing care, education and services compliance while maintaining standards and accreditation status (Joint Commission and CMS). Keeps abreast of changing development, care policies, directions, or practices and the changing education and care needs of the population serviced including state of the art updates in education, management, self-care, devices, medication, prevention, evaluation and research. Responsible for nursing care, education and services compliance with Health Information Patient Privacy Act (HIPPA). Implements and refines strategies to assure billing and collection for case management and education care services for all eligible team members. Formulates and implements the overall case management services program to satisfy varying requirements and priorities for the delivery of those services. Conducts meetings and conferences with subordinate staff to explain work requirements, methods and procedures as needed, Supervises approximately 15 employees who engage in performing a variety of nursing care, technical, and supportive activities for clinical, educational, evaluation, research and community outreach units and procedures for non-routine or complex assignments, new or changed programs, functions, or processes.
