Job Description:

The Caregiver Support Program Coordinator for the Program of Comprehensive Assistance to Family Caregivers (PCAFC) is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence-based services and interventions, program development, caregiver, Veteran, and staff education on caregiver issues, community outreach, resource development, continuous quality-improvement activities and evaluation/consultation related to the PCAFC.
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Social Worker (Program Coordinator). Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: Involve active listening, reading, observing, and evaluating verbal and nonverbal behavior, walking, speaking, writing reports, writing policies and procedures, writing clinical notes, using office equipment such as computers, telephones, fax machines, photocopy machines and travel throughout the Medical Center, into the community to meet community agencies and partners, Veterans, and caregivers. Serve as a gap provider for both same day and short term coverage, which may include the provision of psychotherapy services. Serving as a coordinator for mental health same date access, crisis services, mental health consult triage and disposition process. Work with other clinical staff to triage consultation and treatment requests to Mental Health Programs, including requests for services directly from Veterans, and ensures consults are directed to the most appropriate outpatient MHBS treatment program. Work closely with the MHBS Service Line Manager and multiple MH Program Managers and their designees. Responsible to ensure the efficient and timely scheduling of patients. Coordinating outpatient follow-up for patients discharging from inpatient psychiatric hospitalization and/or residential treatment (i.e., the Domiciliary) which includes monitoring and tracking patients discharged from non-VA psychiatric facilities. Tracking of patients to ensure access to appropriate and timely mental health care and providing enhanced care coordination to Veterans. Providing outreach to Veterans flagged as high risk for suicide and not engaged in MH services which involves working collaboratively with the Suicide Prevention Program. Developing and maintaining a system to ensure follow-up with patients who miss initial consult appointments. Other duties as assigned. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
