Job Description:
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. The Primary Care Physician, in collaboration with the interdisciplinary team, manages the care of outpatients in the Ambulatory Care Services as a Primary Care Provider as part of a Patient Aligned Care Team. Primary Care is provided in Community Based Outpatient Clinic. The Primary Care Physician, in collaboration with the interdisciplinary team, manages the care of outpatients in the Primary Care Service as a Primary Care Provider as part of a Patient Aligned Care Team. This position will float to the Madison East Clinic, Madison West Clinic, Beaver Dam CBOC and Baraboo CBOC. The Primary Care Physician will be responsible for providing comprehensive medical care by performing duties to include, but not limited to the below responsibilities: Deliver direct care and care coordination to the patients in multiple modalities including in-person, virtual, secure messaging and telephone. Provide comprehensive assessment, diagnostic work-up and clinic intervention, including comprehensive patient assessment, health history, physical examination and patient education to veterans in the primary care setting. Manages chronic conditions, evaluates and treats inter-current medical conditions. Performs a variety of disease prevention related activities including gender and age specific screenings, mental health screenings, and health promotion activities. Screens for, evaluates and treats uncomplicated common mental health disorders, including depression and common addictions, with referrals made to mental health providers for further evaluation and treatment as appropriate. Collaborates with a Patient Aligned Care Team (PACT) to provide accessible, comprehensive, coordinated, continuous care to an assigned panel of patients. Hand off communication to and from primary care and specialty departments, emergency department and inpatient providers within the VA and the community. Review non-VA records and incorporating into the ongoing plan of care Primary care providers will act as surrogates for absent primary care providers when needed, to include review of CPRS view alerts. Primary care providers will regular attend primary care staff meetings, departmental meetings and medical staff meetings. Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion. Clinical reminders should be addressed as medically appropriate. Quality of care should meet professional standards at all times. Pay: Competitive salary, annual performance bonus, regular salary increases.
