The VISN 20 Clinical Resource Hub (CRH) is a VISN-owned and governed program focused on increasing access to VHA clinical services for Veterans, especially where local facilities have gaps in care or service capabilities. VISN 20 encompasses Idaho, Washington, Oregon, and Alaska. We are seeking a dedicated PACT/Women's Health Pharmacist Practitioner to join our Primary Care Team.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: ASHP accredited PGY1 and PGY2 in relevant practice area, Board Certification or other applicable certifications, at least 2 years of experience as a PACT or Women's Health CPP, and recent experience as a primary preceptor for students and/or residents. VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Position Overview: The PACT Clinical Pharmacy Specialist (CPS) will be responsible for providing evidence-based pharmaceutical care services through interactions with both patients and providers. This role supports VA sites using both CPRS/VistA and the new Oracle-Cerner Electronic Health Record (EHR) systems. Conducts virtual encounters with patients for CMM in the PACT/Women's Health practice area. Achieves minimum productivity expectations as set forth by the National CRH Leadership Team and Clinical Pharmacy Practice Office (CPPO). Follows-up with patients in a timely manner and monitors for drug therapy response, potential adverse drug reactions, allergies, compliance, laboratory results, and other monitoring measures as appropriate; treatment plan adjustments are made accordingly based on assessed measures. Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, pharmacokinetic impact, and therapeutic outcomes as required; information is communicated with team members and appropriate alternatives to current treatment plans are provided. Promotes and monitors compliance for established drug therapy policies. Coordinates with and refers to appropriate diagnosing providers when disease progression or events occur that are outside the CRH PACT CPS provider SOP. Identifies patients who may benefit from CMM through population management, team meetings, and/or huddles. Documents appropriate subjective and objective information, as well as clinical interventions in the electronic medical record and in a timely, professional manner. Reports and documents drug errors, adverse drug reactions, and allergies according to policy. Conducts drug utilization reviews, quality assurance activities, and utilization reviews as required. Works with health care team members to ensure compliance with national, VISN, and local initiatives. Serves as an authority on drug usage, interactions, and compliance. About the Program: The VISN 20 Clinical Resource Hub employs an interdisciplinary staff, including Clinical Pharmacist Practitioners, serving 8 VAMC facilities throughout the Pacific Northwest. The CRH is committed to providing formal educational experiences, including hosting an ASHP-accredited PGY2 Ambulatory Care residency program, and has affiliation agreements with Idaho State University and 8 VA Medical Center residency training programs.
