Job Description:
This position is responsible for nursing care management services at the Colville Service Unit, Purchase/Referred Care (PRC) program in Nespelem, WA. The primary purpose will be to provide case management, follow-up of continuity, and coordination of care for patients of all ages and those sent to other facilities on Contract funding/Purchase Referred Care

BASIC REQUIREMENT(S): Education: A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing. A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education , Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. Basic Requirements for the GS-11 (or equivalent) Grade Level In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector. Examples type of work and/or tasks: Developing a tracking system for patient care coordination and care management across the continuum; independently planning nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; experience with a wide variety of medical disorders and understands the normal course of disease with anticipated complications and indicated therapeutic interventions, providing care management for patients by coordinating services to meet patient's care goals; ability to communicate effectively with a wide variety of people, including patients, staff or other facilities, nurses, doctors; providing oversight to ensure compliance and appropriateness of care. Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Total Compensation | Pay (ihs.gov) Duties Serves as clinical liaison and resource for the staff for clarifying provider referrals/orders; assist with care coordination and care management of primarily uninsured patients/referrals; Assist with reviewing records and making appropriate referrals, coordinating care and integrating clinical services, coordinating the case management and discharge planning processes, and determining available third-party resources through monitoring patient admissions and transfers. Facilitates communication with staff and partners with the clinical care teams to ensure referrals are appropriate and timely. Arrange specialty appointments and procedures for patients at other facilities, Is a resource on case management/managed care issues for providers, patients and their families by reviewing charts and care plans and contacting patient/family members and service providers as necessary. Manage outpatient case management that consists of consultations from various health care providers to assess the level of care needed and effective uses of resources. Provide education to non-clinical staff on medical terminology and specialty care.
