Job Description:

This Senior Social Worker position as the Patient Aligned Care Team (PACT) CBOC Senior Social Worker is within the Social Work Service at the Burlington Community Base Clinic (CBOC) at the Burlington, Vermont location.
Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: You must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure or Certification: You must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. Exception. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies at the GS-9 grade level. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grandfather Provision: A Social Worker employed in VHA on the effective date of the qualification standard (9/10/2019) who did not meet all the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed, the following provisions apply: Employees may be reassigned, promoted up to and including the full performance (journey) level or changed to lower grade within the occupation but may not be promoted beyond the journeyman level or placed in supervisory or managerial positions. Employees in an occupation that requires a licensure/certification/ registration only at higher grade levels must meet the licensure/ certification/registration requirement before they can be promoted to those higher grade levels. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed on a temporary or permanent basis until they fully meet the basic requirements of the standard. Employees who are converted to title 38 hybrid status under this provision and subsequently leave the occupation lose protected status and must meet the full VA qualification standard requirements in effect at the time of re-entry to the occupation. Employees initially grandfathered into this occupation who subsequently obtain additional education and/or licensure/certification/registration that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. Grade Determinations: GS-12 Senior Social Worker: Experience/Education: The candidate must have at least two years of experience 9 post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. AND Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. AND Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: As a Patient Aligned Care Team (PACT) CBOC Senior Social Worker has experience in case management and practices independently, using professional judgment and expertise to make decisions about services and treatment provided to veterans and their significant others/family members. Candidate should meet the qualification standard for the GS 12 Senior Social Worker as defined in VA Handbook 5005, Part II Appendix G39. This position is part of the Social Work Service Line and is located at one or more outlying Community Based Outpatient Clinic(s) where there is limited access to onsite supervision. As such, the position requires a high degree of independence in practice. Social Work services are located at the White River Junction VAMC and outlying Community Based Outpatient Clinics (CBOCs) and in the communities surrounding each of these locations. The Senior Social Worker possesses advanced practice skills and judgment and has at least two years of experience post advanced clinical practice licensure. The Senior Social Worker is assigned to clinical work for Primary Care as an expert in Social Work. Primary Care encompasses the facility's largest Service Line, seeing the majority of enrolled Veterans at the White River Junction VAMC. The Senior Social Worker is assigned administrative responsibilities for clinical program and intervention development, evaluation and modification. Services provided include serving as the facility's point of contact (POC) for VISN and National initiatives when needed and as assigned by the supervisor. The Senior Social Worker should possess an advanced practice certification or other post-masters training in an area of specialized practice. Duties include but are not limited to: Provides support in implementation of advance care planning, which may include facilitating advance care planning groups; serves as an Advance Care Planning Champion, acting as an expert on this topic and providing consultation and education throughout the assigned CBOC(s). Provides support to the Intimate Partner Violence Assistance Program by serving as an IPV Champion for the assigned CBOC(s); Reviews and supports revisions of policy, procedures and practice guidelines pertaining to service delivery; incumbent will report recommendations as identified; Expands clinical social work knowledge by providing consultation and guidance to colleagues and by role-modeling effective social work practice skills; Mentors less-experienced social workers and collaborates with PACT Lead and Supervisory Social Worker to facilitate the orientation of new employees in assigned area(s), acting as the clinical expert in case consultation and assisting with the acclimation of new social workers at the assigned CBOC(s); Works in conjunction with the PACT Lead and the Supervisory Social Worker to develop best practices that ensure consistency and excellence in social work practice within assigned area(s). Provides immediate psychosocial assessments to individuals and families presenting onsite should urgent needs be required; - Independently completes thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. The incumbent must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable. Documentation of the incumbent's interventions is completed with the national templated PACT social work notes; Identifies and leads inter-professional treatment team discussion on social work needs and actively participates through collaboration with Veterans and family as well as inter-professional treatment team members in the development and implementation of treatment goals and interventions; Performs additional duties as assigned.
