The Lead Medical Foster Home (MFH) Coordinator is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence-based services and interventions, program development, program management, community outreach, resource development, continuous quality improvement activities, evaluation/consultation, and MFH caregiver, Veteran, and staff education.
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Social Worker Program Coordinator: For Social Worker Program Coordinator, grades are determined by the scope of responsibility and the type, size, and complexity of the assignment. Experience, Education and Licensure: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs below. Demonstrated Knowledge, Skills, and Abilities: (a) Skill in coordinating and implementing one or more specialty program(s), across the facility. (b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. (c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. (e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. (f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Develops SOPS for both HBPC and MFH Programs. Manages workload of MFH/HBPC Programs and travels to the 6 clinic locations as needed. Coordinates HBPC SW and MFH services with VA PACT teams, assessing new referrals and admitting new Veterans to the Program. Provides oversight of MFH, SW, and RT services on 9 HBPC PACT teams. Administers information and analytical systems to evaluate and enhance the quality of service provided to caregivers, families and Veterans within the MFH and HBPC Programs. Develops and implements information systems, including clinical guidelines and protocols, to track and establish service activities including visits, referrals, MFH and HBPC caregiver demographics, equipment utilization and caregiver and Veteran satisfaction. Performs work related to the review, monitoring, analysis, planning, and assessment of MFH and HBPC programs. Works closely with senior management and others in implementing and evaluating policies, plans, and internal operational systems such as correspondence management, information tracking, reports and statistics for the facility, VISN and national use. The Lead MFH Coordinator provides direct supervision to a multidisciplinary team covering multiple programs to include the HBPC SW staff, additional MFH Coordinator staff, and HBPC RT staff. Will also provide assistance with supervision/assignment of duties regarding MFH/HBPC PSA. Performs other related duties as assigned.
