The Durham VA Health System, has 1 vacancy for a full-time Registered Nurse for the Treatment Resistant Disorder (TRD) Care Coordinator . The RN will be fully qualified and certified to function in a clinical role and in accordance with the guidelines and policies of the Durham Veterans Affairs Health Care System
Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. The incumbent may occasionally be exposed to patients who" are combative secondary to delirium, dementia, or psychiatric disorders. Must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The Registered Nurse (RN) for the Treatment Resistant Disorder (TRD) Care Coordinator position will serve the Veterans in the Outpatient TRD Program. The patients entering this mode of treatment may have been unsuccessful with therapies for their diagnosis such as major depression, compulsive disorder, eating disorders and some bipolar disorders to name a few. The RN will provide the initial consultation, develop a safe treatment plan with the TRD Provider, schedule the appointment for Electroconvulsive Therapy (ECT), Intravenous Ketamine infusion treatment, Transcranial Stimulation (TMS), and the medication regimen for Esketamine, Clozaril, Lithium or a combination of regimens as needed. The RN for TRD will educate the family, caregiver and veteran on the holistic treatment plan with expected or untoward events associated with the treatments. The RN will work closely with the interdisciplinary team to provide the safest and most effective treatment plan based on individual care needs. The treatments may require lab monitoring, scans, adverse events during treatment and proper intervention as needed. The RN will case-manage the care of Veteran until he can resume a level of care with his primary care provider by using the CPRS documentation system and collecting data to support a response to the treatment given. Preferred Qualifications Minimum of five years successful Mental Health Nursing practice Prior Case Management Experience in the Treatment of Resistant Disorders and motor threshold (MT) determination, procedures and clinical observation. Demonstrated ability to work effectively with patients, families, and other members of the interdisciplinary team in an outpatient setting. Demonstrated skills in decision-making, sound clinical judgment and critical thinking abilities.
