This position is located in the Physical Medicine and Rehabilitation Service, Boise VA Medical Center, Boise, ID.
Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education and Experience: The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. (3) Doctorate degree in physical therapy. NOTE: Prior to 1995 the terminal degree for PT's was either a bachelors or master's degree. In 1995 CAPTE accredited all baccalaureate, master's, and Doctor of Physical Therapy terminal degree programs. Effective 2002, CAPTE changed its scope of accrediting activities of PT education programs to include only those programs that culminate in post-baccalaureate degrees(eliminating the baccalaureate degree). Effective 2015, professional physical therapy education programs have only offered the Doctor of Physical Therapy (DPT) degree to all new students. NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. The CAPTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA) to accredit entry-level physical therapy programs. Verification of accredited programs may be obtained from the American Physical Therapy Association (APTA) at www.apta.org. Licensure: Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Non-licensed PTs, who otherwise meet the basic requirements in this standard, may be given a temporary appointment as a graduate PT at the GS-11 grade level under the authority of 38 U.S.C. 7405 (a)(1)(D) for a period not-to-exceed two years from the date of employment on the condition that such PT provide care only under the supervision of a PT who is licensed. Failure to obtain licensure during that period is justification for termination of the temporary appointment. NOTE. Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetency or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the state for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed physical therapists on VA rolls as of November 30, 1999, provided the individual maintains a continuous appointment and is not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. English Language Proficiency: Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English. GS-13 Physical Therapist (Program Coordinator) Grade Determinations: Experience: In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level. Demonstrated Knowledge, Skills, and Abilities: In addition, the candidate must demonstrate all the following KSAs. 1. Ability to apply advanced knowledge of contemporary physical therapy within specialty area to provide clinical guidance, training or education for internal and external stakeholders. 2. Ability to serve as a consultant within the assigned program area for intra-agency and interagency planning and to provide service coordination to optimize program continuity, efficiency and effectiveness. 3. Knowledge of and skill in management, administration and/or education methodologies. This includes the ability to monitor and track data, utilize available resources effectively, evaluate program quality, and generate reports for local, and/or VISN and/or VACO leadership. 4. Ability to develop and coordinate treatment and/or educational programs within a concentrated field or specialty area, such as, but not limited to, amputee, spinal cord injury, chronic pain, student clinical education/residency programming, polytrauma/traumatic brain injury. 5. Ability to establish and maintain collaborative relationships within department and with other departments or affiliated programs. 6. Physical Requirements: Physical aspects associated with work required of this assignment are typical for the occupation and would generally not require a pre-placement examination. Exceptions: A pre-placement examination is required for any occupation or assignment that requires the operation of a government-owned or -leased motor vehicle in order to properly carry out assigned duties. Examinations may also be required for any occupation or assignment that requires a pre-placement examination by virtue of another policy, regulation, or statute.
Duties include but are not limited to the following: Performs as an independent practitioner within the Boise VA Medical Center, primarily in the outpatient setting with the ability to provide/consult with care in the acute and CLC settings for specialty mobility devices including but not limited to manual and power wheelchairs, offloading devices and specialty lifts as medically necessary for veterans. Performs examinations and evaluations of individuals who have or may develop impairments, activity limitations and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary and/or integumentary systems and uses this information for appropriate device prescription. Provides care coordination across service lines at the Boise VA including triaging consults/referrals related to Wheelchair/Specialty Mobility devices to ensure the timely management of veterans needs based on medical acuity/complexity. Collaborates with PM&R team members, PSAS and other facility partners as well as community care healthcare providers/vendors to optimize care and resource utilization related to mobility equipment. Serves as a consultant/mentor for health care providers regarding the delivery of Wheelchair/Specialty Mobility including departmental and facility level education. Acts as the point of contact to disseminate information from National VA, VISN and other applicable agency directives, initiatives and education. Provides recommendations for device prescription/use and appropriate rehabilitation needs including instruction and education in therapeutic interventions to optimize patient function. Collaborates with PM&R leadership to develop programming, optimize administrative processes and/or obtain equipment/resources related to Wheelchair/Specialty Mobility program. Serves as a consultant/mentor for health care providers regarding the delivery of Wheelchair/Specialty Mobility including departmental and facility level education. Collaborates across the continuum of care. Ensures that services are coordinated, including recommending studies of value and being consumer-centered by referring, co-managing, engaging consultants, and independently supervising care. Uses differential diagnosis as a consistent way to screen for systemic diseases and medical conditions that can mimic neuromuscular and musculoskeletal problems. The model covers past medical history, risk factor assessment, clinical presentation, associated signs and symptoms, and review of bodily systems for each patient. Manages patients with multiple personal factors and/or comorbidities that impact the plan of care. Completes an examination of multiple body systems using standardized tests and measures, addressing multiple elements from any of the following: body structures and functions, activity limitations, and/or participation restrictions. Manages patients with unstable and unpredictable clinical presentations. Exercises clinical decision-making of high complexity using standardized patient assessment instruments and/or measurable assessments of functional outcome. Performs PT evaluations and treatments according to established PM&RS/HBPC protocols and/or evidence-based practice guidelines. Makes autonomous clinical decisions in an autonomous and/or Direct Access environment. Includes the ability to independently evaluate and treat patients who seek physical therapy services. Serves as a subject matter expert for other medical center staff in the evaluation and rehabilitative treatment of individuals with, or at risk to develop, limitations related to cardiovascular, pulmonary, neuromuscular, musculoskeletal, and/or integumentary systems Performs examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and or/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors.
