This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Tect (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations: GS-13 The following criteria must be met when determining the grade of candidates. Experience: In addition to the basic requirements, candidates must have 1 year of experience equivalent to the next lower grade level (GS-12) and demonstrate the following KSAs: 1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. 2. Expert understanding of regulatory and quality standards for their program area. 3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes. 4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. 5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. The Academic Detailing Clinical Pharmacy Practitioner (CPP) functions at the highest level of clinical practice and works under a scope of practice as approved by the Medical Executive Committee of their respective facility. The CPP functions to initiate, modify, discontinue, and monitor medication therapy and acts as a consultant for medication therapy management services. Directly assists other providers in rational medication selection by providing written and/or verbal consultation. Provides medication information to medicine and specialty team members as required. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization. Assists in initiation of medication use improvement strategies including development of quick order sets, clinical reminders, patient/provider education handouts, and development and implementation of pharmacotherapy protocols. Performs data analysis to measure performance, trend changes, and implement quality improvement strategies. Ensure that all prescriptions and actions meet proper eligibility criteria on a patient specific basis and the National Formulary and Non-Formulary procedures are followed. This includes adherence to all national, VISN and local criteria or use treatment algorithms. Assures compliance with VA and other accrediting bodies in terms of meeting all medication use standards. Actively participates in local, state, and national organizations related to the advancement of the pharmaceutical profession. Functions as an advanced practice provider for CMM services to achieve defined outcomes through direct interactions with patients and providers in assigned care area(s). Designs and documents a therapeutic regimen or plan, based on patient specific information that includes establishing a therapeutic endpoint and stop criteria, electing an appropriate drug and related therapy, and establishing an appropriate dosage regimen. Provides consultation on selection of appropriate therapeutic regimens as a follow-up of non-therapeutic drug concentrations obtained from ambulatory care and inpatients. Evaluates drug literature by analysis of experimental design and methodology, in order to compare and contrast therapeutic regimens and roles for new drugs.
