Job Description:
The Social Work Program Coordinator position consists of being the Care Coordination and Integrated Case Management (CCICM) Program Coordinator and providing direct patient care as a Social Work Lead Coordinator as assigned across the New Mexico VA Health Care System (NMVAHCS).

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Licensure: Candidate must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: Work is sedentary but also demands standing, walking, bending, twisting, and carrying light items.
Major duties include: Develops and implements policies and procedures specific to NMVAHCS and ensures compliance with local facility, VISN, the Associate Director of Patient Care Services (ADPCS), Social Work Service), VA Central Office (VACO) directives and handbooks, as well as Joint Commission, and other accrediting bodies and regulatory standards as needed. Collaborates with nursing counterpart, for the planning, development, implementation, monitoring, and evaluation of the CCICM operations. Plans, organizes and directs administrative, operational, Veteran, and personnel activities for the service, in conjunction with the Service Chief and/or supervisory designee and nursing counterparts. Identifies cohort of Veterans across NMVAHCS with the highest opportunity and highest need. Identifies potential areas where care coordination is taking place to include but not limited to primary care, surgical care, mental health, community care, specialty clinics, geriatrics, and chronic disease. Integrates inpatient and outpatient care coordinators, care managers, and case managers, for seamless continuity of care. Ensures compliance with policies and standard operating procedures for evaluating and mitigating suicide risk to include completion of the Columbia Suicide Severity Risk Score (C-SSRS), Comprehensive Suicide Risk Evaluation (CSRE), Safety Plan, and Overdose Report as clinically indicated. Conducts psychosocial assessments to determine needs of veterans and family members. Participates in treatment planning process, including discharge planning with other disciplines. Provides case management to veterans and families, as needed, throughout the continuum of care. Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
