Job Description:

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated and ventilated. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations.
Total Rewards of a Allied Health Professional The Social Worker (Program Coordinator) Hospice and Palliative Care Program Coordinator will work within the Medicine Service Department. He/she is responsible for implementation of the Palliative Care program; ensuring high quality practices in treatment of symptoms at the end of life through the coordination and integration of various nursing, medical and complementary interventions. This responsibility includes coordination of the efforts of the multiple disciplines involved in caring for patients at the end of life in the inpatient supportive care unit and the Palliative Care Consult Team; serving as a consultant and resource person; development and implementation of educational programs; and development and implementation of specific measures to identify cases and monitor the effectiveness of the program. Duties include but not limited to: Program Management Facilitates and promotes the delivery of high quality care to patients at the end of life and their intimate others and well as significant others; Responsibility for the continuing development, management, implementation and supervision of program operations; Screening/Assessment Receives and completes consults from interdisciplinary teams, inpatient and outpatient providers, and other social workers on complex, difficult cases, using advanced practice skills and expertise; Conducts on-going review of individual treatment plans and evaluation of treatment effectiveness and ensures that initial assessment interviews, evaluations, and treatment are in accordance with established policies and current guidelines; Treatment Planning/Goal Setting Develops psychosocial treatment plans in collaboration with the Palliative Care interdisciplinary team, including goals for psychosocial clinical treatment; Provides independent consultation and makes recommendations to the interdisciplinary team regarding the course of treatment; Selects and uses group, individual, conjoint-family and community treatment interventions and methods within his/her scope of clinical proficiency as appropriate to the unique, individualized circumstances of each patient in order to achieve these goals; Crisis Intervention Making rapid assessments and developing crisis management plans to address any issue that impacts patient care. In the event of an alleged abuse case, medical or psychiatric emergency, the medical center policy will be followed; Administration Responsibilities Interviews and recommends personnel. Plans and organizes the work to meet workload demands and to equalize caseloads among staff members; Receives and completes requests for services (consults) from interdisciplinary team members and from other Social Workers on complex, difficult cases, using advanced practice skills and expertise; Uses initiative and resourcefulness in deviating from traditional methods or trends and patterns to develop new methods, criteria, and proposed new policies. Practices at an advanced level and independently gathers relevant information to guide practice, including community Social Work standards.
